Caring for My COPD 

Caring for My COPD is a 10- week pulmonary rehabilitation program for people who have been diagnosed with Chronic Obstructive Pulmonary Disease (COPD) including those recently hospitalized due to COPD 

If you have COPD, consider attending our community based program to help you gain knowledge about your disease and better control of your COPD.

  • There is NO COST for the program
  • Weekly education sessions that will help you understand & manage your COPD
  • Personalized, supervised exercise sessions
  • Your family member(s) or support person are welcome to accompany you to education classes
  • You will have access to COPD Coordinator and all other members of the COPD Healthcare Team
    • Kinesiologist, Social Worker, Occupational Therapist, Physiotherapist, Dietitian, Certified Respiratory Educator.

Speak to your Physician or Nurse Practitioner to complete a referral for you to attend. You are welcome to refer yourself to the program and we will contact your physician to complete a referral.

Nutrition Education

The impact diet and nutrition has on your overall health is no mystery. Compass CH understands that you may have concerns about your diet, questions about what is 'healthy' or simply wish to make a change in your diet, but don't know where to start. The Registered Dietitians (RD) at Compass CH are here to help by providing nutrition counselling and education to enrolled patients.

What is a Dietitian?

Registered Dietitians (RD) provide nutrition education to clients based on evidence-based knowledge of food and human nutrition. Due to the influence of social, cultural and economic factors on your relationship with food, RDs integrate this knowledge with that from other disciplines, including mental health, health and social sciences and rehab.

Who is eligible to see the Dietitians at Compass CH?

  • Any Hamilton region individuals who have type 2 Diabetes or Pre-diabetes
    • If the individual's doctor is part of the Hamilton Family Health Team, the individual must ask for their Doctor’s permission first
  • Any individuals (with or without diabetes) whose family doctor practices at Compass CH
  • Individuals accessing the Caring for My COPD program

Our dietitians counsel people and offer nutrition classes mainly in the field of diabetes. 

However, if you are a primary care client, you can book to see a dietitian at Compass CH on a wide range of topics, including:

  • Weight management
  • Prediabetes and prevention
  • High blood pressure
  • High cholesterol
  • Osteoporosis
  • Stomach or bowel problems (i.e. irritable bowel syndrome, celiac disease, Crohn's and colitis, diverticulosis)
  • Kidney disease and kidney stones
  • Food allergies and intolerances
  • Vegetarian and vegan diets
  • Youth nutrition
  • Nutrition during pregnancy
  • Sports nutrition
  • Cancer
  • Senior wellness and health
  • Picky Eaters

Diabetes Education

Our Diabetes Program will provide education, tools and the ongoing support for clients with Type 2 and prediabetes.  

Your Diabetes Team

Your Diabetes Team consists of Yourself, your Family Physician, Certified Diabetes Educators, Pharmacist, Chiropodist, Social worker and Kenesiologist. 

What Services are Provided by the Diabetes Program?

Clients are seen by the Registered Nurse and/or the Registered Dietician, whom are Certified Diabetes Educators.  Ongoing education through individual or group classes will assist client with the tools to self-manage your diabetes. Through our ongoing discussions we may feel you could benefit from a referral to another provider on your Diabetes Team.

Group Education

We currently have Diabetes Kitchen, Diabetes Exercise Program and Self-care group that covers Diabetes Management and Foot care.

Self Care group is currently being run throughout the calendar year with 5 sessions posted for this season

Diabetes Kitchen 

Open to people living with diabetes, their families, or those who have been told they have prediabetes.

Compass Community Health (438 Hughson St. North) Community Kitchen

Meeting Times

1 Friday per month for 3 months 10:00 am - 12:30 pm

  • September - November
  • January - March
  • April – June

Goals of Program:

  • food safety education
  • Improve confidence in the kitchen
  • Cook more to limit processed food and restaurant use – Save money!
  • Show how to make healthy choices taste great

Focus on learning opportunities to increase confidence in the kitchen: 

  • Knife skills
  • Browning skills
  • Healthy alternatives
  • Vegetarian protein options
  • Money saving tips, learn how to use a Flyer matching App

Diabetes Exercise Program

Individual Appointments

At your individual appointment with your Certified Diabetes Educator, your current health status will be reviewed. This includes:

  • Discussing your diet and nutrition
  • Reviewing your medications and medical history
  • Taking measurements of health indicators, such as your weight and blood pressure
  • Assessing current usage of blood glucose monitor

You and your team will work together in setting lifestyle goals that will aid in your progress towards effective diabetes management.
